MISSION TRAJECTORY

The Health Apostolate Commission aims to strengthen the healing ministry of Christ by promoting holistic health, dignity, and compassionate care for all, especially the poor and vulnerable. It focuses on preventive healthcare, awareness, and accessible services through trained volunteers, structured teams, and collaboration with healthcare professionals.

The Commission envisions a fully active health ministry in every parish, reaching the entire diocesan community with care, accompaniment, and hope.

  • Establish Diocesan Health Core Team and form Deanery Health Teams.
  • Launch parish-level health volunteers (minimum 3 per parish).
  • Conduct training programs on home-based care, elderly and pastoral care.
  • Begin registration and regular visits (every 2 months) for elderly/sick.
  • Initiate health directory and referral linkages.

Medium Term

2029-2031Consolidation
  • Expand to 50–100% parish health apostolate units.
  • Implement a structured Sick & Elderly Ministry across parishes.
  • Strengthen referral systems and partnerships (hospitals/clinics).
  • Ensure regular outreach services in remote areas.
  • Increase coverage of patients and vulnerable groups significantly.

OBJECTIVE 01

Forming Health Apostolate Teams.

KPIs

  • By Dec 2026, form the Diocesan Health Core Team (10–12 members).
  • By June 2027, 100% of Deaneries (5) have Health Teams (6–8 members).
  • By Dec 2028, 50% of parishes have Parish Health Volunteers (min 3/parish).
  • By Dec 2033, 100% of parishes have active Health Apostolate Units.

OBJECTIVE 02

Strengthening Compassionate Care Ministry for Elderly, Sick, and Remote Faithful.

KPIs

  • By Oct 2027, conduct 5 deanery trainings on home-based and elderly care.
  • By Jan 2028, 400 elderly/sick persons registered and visited every 2 months.
  • By Dec 2028, 10 remote mission stations receive annual health outreach.
  • By Dec 2033, 1,500 vulnerable persons receive continuous care.

OBJECTIVE 03

Establishing a Decentralized Diocesan Health Support and Referral Network.

KPIs

  • By June 2027, prepare a Health Directory (40 doctors, 60 nurses, 80 volunteers).
  • By Dec 2027, each deanery links with at least 2 hospitals (10+ referral centres).
  • By April 2028, 50% of parishes implement a basic referral system.
  • By Dec 2028, 300 patients/year supported through referral services.

OBJECTIVE 04

Ensuring Medical Outreach and Compassionate Care Services.

KPIs

  • By Sept 2027, 10 medical camps annually in rural areas (2,000 beneficiaries).
  • By April 2028, all 5 deaneries conduct at least 1 major annual health camp.
  • By Dec 2028, 500 elderly/sick persons enrolled in pastoral care visits.
